Your first session is a 2-3 hours and includes paperwork and a full psychiatric evaluation. It is charged at $362.25. There is a 3.5% reduction for cash. During this appointment you will be completing a lot of paperwork that will allow me to pinpoint the issues more expediently. This is a full psychiatric evaluation during which the causes of your suffering will be teased out – what is biological and what is psychological – and further, what can be done about these causes. You will be given a receipt ("super bill") which will contain all necessary information for you to file a claim for out of network benefits with your insurance company, unless you have medicaid or medicare. If that is the case, I will be asking you to sign a contract for paying me directly. You would not be able to submit the claim to Medicare or Medicaid. Before you make an appointment, please check with you insurance company to find out what you benefits are for out of network mental health services.

The follow up appointment take between 1/2-2 hours. They are usually cost between $207-458.75. These session may include medication management, psychotherapy, and/or meditation/hypnosis. The maximum time between appointments for a stable client is 3 months. Telehealth options available. As with your first appointment, you will be given a receipt for filing an out of network mental health services claim, unless you have Medicaid or Medicare as above.
Effort will be made to time your follow up appointments with medication renewals. This allows for required documentation to be completed. For medication renewal in between appointments - please text requests to 607-624-1775 at least 4 business days in advance. You must have had an appointment in the last 3 months and have another one scheduled.
Dr. Belsare does not accept any type of insurance. We can provide you with a receipt to bill your insurance by filing an out-of-network claim form. You could then download a claim form from your insurance website, fill out with the numbers on the receipt, and send into your insurance. Each insurance program is different. Most do refund a portion of your cost or apply it to your deductible. This does not apply if you have Medicaid or Medicare (including any government sponsored insurance such as Child Health Plus or Managed Care). In that case, you can still enter treatment but you would be responsible for paying out of pocket directly. If you have public insurance, you would not be able to submit a claim for re-embusement.
If you cannot afford paying directly, please consider CTRC, which accepts all insurances, at 607-797-0680 or Aspect Psychiatric Services in Vestal. Substance use treatment can be obtained at ACBC, the Addictions Center of Broome County.
With rare exception, I do not accept individuals below the age of 16.
There is no charge for cancellations made with 24 hours notice. For later cancellations, $50 will be deducted. If you do not cancel and do not attend your appointment, you are liable for the full session charge.
